Immunizations are an excellent way to protect yourself and your family against many diseases. Age-appropriate immunization starts at birth and continues through adulthood, with special emphasis through age two. Your health care provider can help you determine which immunizations you or your child may need.
Click here to download a blank Certificate of Immunization Status (CIS) Form. This form must be filled out by a parent/guardian. The school will not accept a blank one or one that only has a signature with no dates filled in.
Shots are required by state law for children in attendance at public and private schools, preschools, child care facilities, and Head Start programs in Oregon. Nearly every place that provides care for a child outside the home requires shots or a medical or nonmedical exemption to stay enrolled. Religious exemptions signed before March 1, 2014 are no longer accepted. Parents will need to submit new proof of immunization or a nonmedical exemption for their children if their nonmedical exemption was signed before March 1, 2014. If parents fail to update this documentation, the child will be reported as "incomplete" or "no record" and may receive an Exclusion Order.
Two doses of Hepatitis A are now required for all children in kindergartens and grades 1-8.
Registering for Kindergarten: When registering a child for kindergarten, families must show immunization records demonstrating that their child has had the immunizations required by the state of Oregon. You can request a copy of those records from your child's physician and then transfer them onto the Oregon CIS Form. Again, the school will not accept a blank CIS Form or one that only has a signature with no dates filled in.
If you're registering in the spring, you'll have time to schedule the vaccinations before school begins. Keep in mind, however, that before starting school, you must be able to show that your child has received at least the first vaccination in any series of vaccinations. Families should schedule the required additional vaccinations as quickly as possible.
Deadline for Full Immunization: At the beginning of each year, per the Oregon Immunization Program, school staff begin reminding parents that children will not be able to attend school or child care starting the 3rd Wednesday in February (2/15/2017) – School Exclusion Day – if their records on file show missing immunizations. The Health Services staff work with schools to make every effort to contact families to remind them of this deadline and provide information about resources that may be available.
